A supplier sends a scanned PDF, and your AI summary leaves out a delivery condition that you can see on the page. Before rewriting the prompt, check the document that actually reached the workflow and the text it contains.

Separate three questions: is this the right file, is the relevant content available in usable form, and does the summary accurately reflect it? A successful upload answers none of those on its own. This checklist helps you locate the missing evidence without claiming that every AI tool handles PDFs the same way.

Check the exact source and a known phrase

Open the file your workflow received, not a similarly named attachment elsewhere. Compare its revision and page count with the sender’s intended document. If an attachment was replaced or only selected pages were submitted, settle that before investigating the model’s answer.

Choose a short phrase you can clearly see on the page. Try searching for it in your PDF viewer, and inspect what happens when you select and copy a small passage into a safe local text editor. Does it produce the expected words, nothing, or a different sequence?

Treat this as a diagnostic sample. One successful search does not prove that every page has usable text. One failed search does not prove OCR is the only problem; document permissions, viewer behavior or another processing step may need investigation. Do not disable security controls to force the test. Ask the document owner for an approved accessible version when necessary.

Record the file reference, PDF page position and observed result. Keep printed page labels separate from PDF page positions so another reviewer can find the same passage.

Adobe advises reviewing OCR output for accuracy and completeness. Apply that advice before an AI summary becomes an operational instruction. Compare a few consequential fields with the visible source: a date, reference code, quantity or condition that changes the next action.

A matching sentence does not cancel the reference-code mismatch or establish coverage of the last page. Keep those findings separate. Correct or investigate the affected text through the approved process, then repeat the same check. Preserve evidence of the original discrepancy instead of quietly replacing the source record.

If your workflow exposes its extracted text, inspect that artifact too. The working PDF may contain corrected text while the automation still uses an earlier extraction. Compare the actual input revision and content; do not assume rerunning a summary refreshed every upstream step.

Ask for evidence before accepting the AI summary

Once you have a checked input, ask for a bounded answer with traceable support. For a synthetic training document, a prompt could say:

Summarize only the supplied document. List delivery conditions separately. For each condition, provide a short supporting passage and its page reference if available. Mark missing or unreadable sections as unknown. Do not invent a condition, approve an order or contact anyone.

Check the supporting passage against the source yourself. A plausible citation is not verification. If the tool cannot provide reliable page references, record that limit and use a manual comparison rather than pretending the trace exists.

Keep the summary in draft status when a consequential phrase remains unreadable or unsupported. The next owner needs the exact file revision, affected page and unresolved question—not merely a polished answer. Document which pages and fields were checked; do not describe a sample as a complete-document audit.
